Does OsteoMed participate with any insurance companies?

OsteoMed II does not currently participate with any insurance company, HMO, PPO or managed care plans. You are responsible for payment of fees at the time of service. We accept CASH, CHECK, MASTERCARD, VISA and DISCOVER. We do not accept American Express.

You will receive a statement, called a HCFA form, detailing your treatment for each visit; the form contains all the information needed by your insurance carrier, including a doctor's signature. Be sure to keep a copy for your records.

You are advised to contact your insurance carrier prior to your appointment.

Ask the following questions:

1. What coverage do I have for "out-of-network" office visits with a doctor? What percentage of the bill will be paid?

2. What is my annual deductible? How much of that amount have I met? (after your deductible is met, your insurance will pay 80:20, or the amount normally paid for medical expenses with an out-of-network provider. Your deductible must be met before you receive ANY reimbursement)

3. Does my plan cover ________? (examples: acupuncture; prolotherapy; osteopathic manipulation; out of network lab tests)

Most of our patients with private insurance have 30-80% of their services covered AFTER their deductible is met.

As of June 1, 2003 we no longer participate with Medicare. Medicare patients can obtain care at OsteoMed by signing a "Private Contract for Medical Treatment," which means you will pay for your services and you WILL NOT be able to submit to Medicare or a Medicare supplement for reimbursement.

OsteoMed II does not participate with Medicaid. Medicaid patients may private contract with the clinic for services through special arrangements only. We must strictly adhere to the government's rules regarding Medicare and Medicaid patients

You may want to consider CareCredit. Through this service you can receive all of your care within weeks and have one year at ZERO INTEREST. Any reimbursement you receive from your insurance can be applied to your Care Credit balance. Apply today from the privacy of your own home!
